Fungus Free Nails-Rx.
  1. Fungal Free Nails, (FFN) a combination of 12 fungicidal essential oils with a patented carrier system is the only topical treatment for onychomycosis that works. It penetrates the nail plate via the intercellular lipid layers between the dead corneocytes and reaches the nail bed where the fungus resides.
  2. The initial response rate to FFN is over 90%. There are relatively few cases of resistant trichophyton, the causal organism.
  3. FFN needs to be applied twice daily. We tell patients to keep the vial at their bedside and apply it upon awakening and before going to sleep. Even though the nail may appear wet for up to 45 minutes, it has effectively penetrated in 3 minutes or less, after which it is okay to take a bath or a shower, or put on socks.
  4. Reasons for initial failure include:
    • Non-compliance; (50% non-compliance rate in U.S. all conditions, all meds).
    • Continued microtrauma to toenails (ill-fitting shoes).
    • Other than fungal etiology of pathologic nails.
    • A totally dystrophic nail with involvement all of the way back to the matrix. This nail plate is totally lifted off of the nail bed and FFN cannot penetrate to the nail bed because of the gap.
  5. Initial success, witnessed by, in the first 30 days:
    • Thinning of the diseased nail.
    • A distinct demarcation line between normal and diseased nail.
    • Dark or black spots and/or streaks in diseased nail (killed fungal elements).
    • This is best documented by a digital photograph prior to onset of treatment and another one after 30 days of treatment.
  6. Length of treatment is dependent on the total surface area of diseased nail and the patient’s age. The older the patient, the slower the growth of new healthy nail. FFN does not transform diseased nail into healthy nail. By killing the causative fungus, it hastens the crumbling of the diseased nail over time and allows it to be replaced by new healthy nail growth. Treatment time may vary from several months to as much as eighteen months.
  7. Trichophyton, like most fungi, has a life cycle that occurs in two stages: an active pathologic form (hyphae) and a spore form. The hyphae produce spores which germinate over time to produce more hyphae. The spores are impervious to all medications. They exist in both the nail plate and the nail bed. Any treatment regimen has to recognize this and react accordingly. We recommend that FFN be used twice weekly for one year after the nail looks completely clear to kill any residual germinating spores. Our recurrence rate with full compliance is essentially zero.
  8. We now have available an additional formulation for onychomycosis: FFN-Rx. This formulation incorporates cyclopirox, the active element of PENLAC®, and terbenifine, the active ingredient in Lamisil®, and fluconazole, a potent killer of yeast, into FFN so these three medicinals penetrate into both the nail plate and the nail bed.
  9. The appropriate treatment algorithm is as follows:
    • Primary failure: No sign of efficacy in first 30 days, start FFN-Rx. In these cases the first vial will be supplied by us free of charge (this assumes full compliance, no microtrauma to toes, etc.).
    • Secondary failure: Initial success but no further progress, i.e. probable development of resistance, patient needs FFN-Rx available from R&S Research.
  10. If the nail is totally dystrophic, i.e. diseased down to the matrix, it must be continually debrided, and FFN applied to the nail bed twice daily until the new nail grows out completely. There is no change in treatment regimen after nail removal. The fungus, in both active and spore form still resides in the nail bed.

Q: Can I use Fungal Free Nails on my fingernails?

Q: Can the Fungal Free Nails treatment be used in combination with laser treatment for nail fungus?

Q: Why hasn't anyone found an inexpensive effective solution for this problem until now?

A: The simple answer is that in the greatest majority of people the condition is purely cosmetic and the major pharmaceutical companies have historically concentrated on finding solutions to medical problems that threaten loss of function, loss of limb, or loss of life.

Q: Clearly as a society we are generally concerned with having a normal healthy appearance. With so many people suffering from this condition isn't the potential market large enough to attract a major effort from some entity?

A: You would think so. However, nail fungus is extremely hard to eradicate. The most common organism that causes the condition, Trichophyton rubrum, gets into the nail bed of the toes. Oral prescription medications which have to be given in doses that reach a therapeutic blood and tissue level, are expensive, have a definite side effect profile which, although not of frequent occurrence, is potentially dangerous, and many times either doesn't cure the problem or if it does, the problem recurs (comes back).

Q: If oral meds are not the answer, what about topical medication?

A: Interestingly, the only Federal Drug Administration approved topical medicine for the condition of nail fungus is PENLAC which, according to its manufacturer, claims to completely cure this condition in only 5.5% of the cases in which it is used. If you look through the package inserts or the Physicians Desk Reference (PDR) for any other product being sold as a topical antifungal agent to my knowledge you cannot find one that has an indication for nail fungus.

Q: Why is that?

A: The simple answer is that none of these products have satisfied the FDA’s requirement that they prove their worth according to established scientific criteria.

Q: Many of these products contain ingredients that are known to kill fungus. How come they don’t seem to work?

A: The reason that these antifungal products work poorly if at all is that they are put into a water solution and applied to the nail. Since the nail is made of keratin, which in a much less thick layer also forms the outside layer of the skin, and keratin is not penetrable by water the antifungal compounds don’t get to where the fungus resides, which is the nail bed. If keratin could be penetrated by water it wouldn't be safe for us to take bath since we would absorb water through our skin and “blow up.” This obviously doesn't happen.

Q: So how do we get antifungals through keratin?

A: Keratin is does not contain living tissue cells so it can’t actively transport any molecules. It does however have lipid or fat channels that allow certain fat soluble molecules to pass through. These types of fat soluble molecules exist in nature and fall into the chemical family that includes certain organic solvents as well as essential oils. These solvents enhance the diffusion or penetration of the essential oils in Fungal Free Nails that have been shown to kill nail fungus.

Q: Can these other non-carrier essential oils do the job of eliminating the fungus that causes fungal nails?

A: Yes! There are numerous naturally occurring plant oils that are known to have antifungal properties and that have been shown in scientifically published studies [Journal of Photography] to kill the fungus that causes fungal nails.

Q: Has Fungal Free Nails been shown to penetrate keratin?

A: Most definitely. We tagged Fungal Free Nails with Turmeric, a colored food additive and demonstrated that Fungal Free Nails penetrated the keratin of horse hooves at ten times the rate of PENLAC. We also demonstrated almost instantaneous complete penetration of cadaver nails in a leading University Laboratory in California. This laboratory is the reference laboratory (the Gold Standard) for skin and nail penetration in the United States.

Q: Some other products claim to work in days or several weeks. Is that possible?

A: Unless these products contain some magical ingredient or combination of ingredients that accelerates the growth of normal healthy nails by a factor of five to twenty times, it is not possible. Normal nail grows very slowly. Depending on how much of the total surface of the nail is effected by nail fungus, the process of restoring the appearance of the nail to its normal clear condition may take up to one year.

Q: Doctors know that it is very hard to get patients to do any therapeutic maneuver such as take a pill or apply a topical remedy with great regularity. How can we overcome this when we have to apply Fungal Free Nails twice a day for up to one year?

A: The easiest way is to keep Fungal Free Nails at the bedside and apply it before getting out of bed in the morning and before going to sleep at night.

Q: What can I do to help Fungal Free Nails do its job?

A: The fungus that causes this problem thrives under warm, moist, low oxygen conditions. Make sure that your shoes fit properly and are well ventilated, and that your feet and toes are dry. Shoes that are too small will traumatize the nails and keep them from improving.

Q: What should I do if Fungal Free Nails doesn’t seem to be improving in spite of doing the above?
